If you are booking a Functional Medicine New Patient Visit or an ASD Biomedical New Patient Visit, you will be required to pay a deposit of $125 at the time of scheduling to reserve your appointment. This amount is a deposit and serves as partial payment toward your initial visit, and the remaining portion is due on the day of your appointment. **PLEASE NOTE, A DEPOSIT IS NOT REQUIRED FOR NEW OR EXISTING PATIENT CHIROPRACTIC, ACUPUNCTURE, CRANIOSACRAL, FOOT REFLEXOLOGY, ADVANCED INTEGRATIVE BODYWORK OR SMOKING CESSATION APPOINTMENTS. Once you schedule, this $125 reservation deposit will be charged to the card you place on file. We understand that schedules change and emergencies arise, so If you must reschedule your NEW PATIENT appt., we require that you provide the office with ample notice. A notice of 72 hours is required for all FM and Biomedical ASD New Patient visits, and a a 24 hour notice is required for all other appts. If required notice is provided, your reservation deposit of $125 will be reapplied to your newly scheduled appt. date. Any late cancelled or no-show NEW PATIENT appointment will result in a forfeited deposit. Please note, Saturday and Sunday do not count toward the 24 or 72 hour notice as we are not open on weekends. For example, If your appointment is on Monday, cancellation of a new patient visit would be required by the previous Wednesday for 72 hours requirement to be met. We do take our cancellation policy seriously as it is in the best interest of our patients and our practice. We have reserved a large appointment time for you, and any vacancies in the schedule prevent us from helping another patient who could have taken that appointment. Dr. Noah Erickson, D.C., Med. Ac. is the founder and clinical director of Enliven Functional Medicine with locations in both Wexford PA and Boardman OH. He is a graduate of California University of Pennsylvania with a Bachelors Degree in Athletic Training and Logan College of Chiropractic with a Bachelors degree in Human Biology and a Doctor of Chiropractic degree. Dr. Erickson is also a licensed medical acupuncturist through the International Academy of Medical Acupuncture. In addition to chiropractic and acupuncture, Dr. Erickson has had advanced study and training in integrative medicine as well as in laboratory and genetically-based nutrition. This background qualifies him to address health problems using customized dietary protocols and the highest quality nutritional supplements based on specialized lab work and genetic testing. He is one of the few practitioners in the country who specializes in Epigenetics and Nutrigenomics: A biomedical, genetically-based treatment for autism, ADD, ADHD and related disorders.
